It's very possible that the problem is in either or both of these mod_plsql extensions:
mod_plsql 9.0.0
mod_plsql 9.2.0
I have not tested the mod_plsql 9.2.0 installation with Oracle Database 12c, but it may help you to test it. The mod_plsql 9.0.0 installation (with Oracle Database 10g) is available at and the mod_plsql 9.2.0 installation (with Oracle Database 12c) is available at
